1. Services Provided
The Form Friend offers support with paperwork, including (but not limited to) Disability Living Allowance (DLA) applications and renewals, EHCP applications, appeals, Blue Badge applications, and general admin. I do not provide legal advice and cannot guarantee the outcome of any application or appeal. Where possible, I reference and apply relevant laws, statutory guidance, and legislation to strengthen your application.
2. Payment
- A 50% deposit is required to secure your booking.
- The remaining balance is due upon completion of the agreed service.
- Payments are taken via bank transfer (details provided at booking).
3. Cancellations & Refunds
- Deposits are non-refundable once work has begun.
- If you need to cancel before I start, I will refund your deposit in full.
- If you wish to cancel after work has started, only the remaining balance will be waived.
4. Confidentiality
All information shared with The Form Friend is treated as strictly confidential. Documents, forms, and personal details will never be shared with third parties without your permission, except where required by law.
5. Privacy Policy
- Your data is used solely for the purpose of completing your forms or related support services.
- Personal details (such as names, addresses, and medical information) are stored securely and deleted once no longer required.
- I will never sell or share your personal data.
6. Limitations
- I do not attend tribunals, assessments, or meetings in person.
- My role is to assist with applications and paperwork — the final responsibility for submission and accuracy of information rests with the client.
